Choosing the Right Rhinoplasty Approach

Many people ask me whether nose surgery should be done with an open or closed technique. Hello - I'm Mo Anwar, a Rhinoplasty specialist. In this video, I'll explain your options and how the shape of your nose, particularly the middle and lower parts, will determine the best procedure for you. If your concerns are only about a bump on the bridge of your nose, or a mild twist in the bone - I usually suggest the closed approach. This technique lets me make changes without making any cuts on the outside of the nose. However - if there's a prominent cartilage bump, removing it with a closed approach, might make the bridge of the nose collapse slightly. Resulting in a dip or a faint shadowy line down the middle. To stop this from happening, I often use something called spreader grafts. These are small pieces of cartilage, placed inside the nose - to keep the shape smooth and strong after removing the bump. When we need to reshape the tip of the nose, I usually prefer the open approach. This gives me a clear view and better control to make precise changes. This will all be assessed, and discussed during a consultation. In some simple cases - like just needing a bit more support for the tip. I can do that through a closed approach, using a tiny piece of cartilage for support. These photos provide some examples of recent results with this technique However - if the tip is wide; uneven; too droopy; or turned up too much - we usually needan open approach to reshape it for a natural look. Some of these results can be seen below. Fixing noses that have already had surgery before is more complex. In most of these cases, I use the open method so I can clearly see and fix any damage or scar tissue. Very occasionally, if it's just a small issue from a previous surgery, I can fix it with a closed approach. Especially in older patients - with softer nose tissue. In the end, I'll always choose the method that gives you the safest, and most natural result. My goal is to help you feel confident - and happy with your new nose.
